Report: Lorain County Officer Ordered Illegal Cavity Search on Inmate on Suicide Watch

An investigation by the Office of the Corrections Inspector General has concluded that a Lorain County corrections officer ordered and conducted an unauthorized body cavity search on an inmate who was on suicide watch, according to official records.

The incident occurred during the overnight hours of November 9 into November 10, 2025, inside the Lorain County Jail. The inmate, whose name is being withheld due to the nature of the allegations and documented mental health status, had a history of behavioral issues and was frequently placed on suicide watch.

According to the investigative report, jail staff were responding to repeated issues involving soap being smeared over the inmate’s cell window, obstructing visibility. Officers entered the cell to locate the soap and restore visibility.

During the encounter, the report states that Corrections Officer Lori Blaze instructed the inmate to comply with actions consistent with a body cavity search. The inmate initially refused but was ultimately directed to “squat and cough,” while the officer conducted a visual inspection of the inmate’s genital area.

Investigators cited this audio as key evidence in the case. Portions have been edited to protect the identity of the victim.

The investigation found that this type of search was not authorized under policy or law.

Under Ohio law, body cavity searches require a search warrant or documented medical necessity and must be conducted by qualified medical personnel under sanitary conditions. The report found no evidence that these requirements were met.

The inmate later reported the incident to another officer, prompting a chain-of-command notification and a formal investigation.

As part of that investigation, officials reviewed written statements, interviewed staff and the inmate, and analyzed both overhead camera footage and audio recordings captured from inside the cell.

While the overhead camera footage did not clearly show the interaction due to its angle, the audio recording captured from the cell intercom system provided key evidence.

According to the report, the recording includes commands consistent with a body cavity search. Investigators concluded that the audio directly contradicted statements made by Officer Blaze during the investigation.

Officer Blaze had denied instructing the inmate to perform a “squat and cough” or conducting any form of cavity search. However, investigators determined those statements were not truthful based on the recorded evidence.

The report also noted that other staff members appeared uncertain about proper search procedures, and that less experienced officers may have relied on Blaze’s direction, contributing to the situation.

The findings state that the conduct was not an isolated misunderstanding but part of a broader breakdown in adherence to policy and procedure.

Based on the totality of the evidence, the Office of the Corrections Inspector General concluded that an illegal body cavity search did occur and that Officer Blaze attempted to conceal the conduct during the investigation.

As a result, the report recommends that Blaze be separated from employment with the Lorain County Sheriff’s Office and that criminal charges be considered.

The report outlines several potential violations of Ohio law, including unauthorized search, falsification, dereliction of duty, and interfering with civil rights.

It also cites violations of internal policy, including conduct unbecoming, dishonesty, and failure to follow established procedures.

In addition to disciplinary action, the report recommends that the sheriff’s office implement additional training for corrections staff regarding strip searches, body cavity searches, and proper procedures.

Officials also recommended developing an individualized management plan for the inmate involved to better address ongoing behavioral and mental health concerns.

The investigation concluded on January 15, 2026, with officials stating that all reasonable investigative leads had been exhausted and that sufficient evidence exists to support a final determination.

Criminal charges have since been filed in connection with the incident. According to reports, Blaze is facing multiple misdemeanor charges and has been placed on paid administrative leave pending the outcome of the case.
